District Convention Registration ENDS TOMORROW May 5th





Registration form must be received no later than tomorrow, May 5, 2022.  

The registration form is a fill able pdf, download it, fill it in, and email it to lions4service@gmail.com – include your check number in your email. Mail the check to District 4-C4 Convention, P.O. Box 25301, San Mateo CA 94402. Click here for the form.. Your registration form will be held pending your payment - payment must be received no later than May 10, 2022.


Rooms – Rooms at the Convention are contracted for no more than 2 individuals per room. Meals included are for the 2 registered individuals ONLY.


Meals – If you are not staying at the hotel, you can purchase Friday Lunch, Friday Dinner, Saturday Dinner – as listed on the registration form due May 5th with payment to be received no later than May 10, 2022.


Name Badge – If you are not staying at the hotel and you are not purchasing meals, you must register to attend the events/meetings. There will be chairs in the rear of the ball room for you – tables are reserved for Lions/Guests who purchased meals. Complete the top portion of the registration form and mark the “Name Badge Only” box and email to lions4service@gmail.com – there is no charge for a name badge.


All attendees/delegates must have a NAME BADGE – Everyone must wear a name badge to attend all events


Delegates –

  • Club Delegate Forms must be received by 5:00 PM on Thursday, May 19th
  • Delegates must be certified by 10:00 PM on Friday, May 20th
  • Voting is Saturday, May 21st between 7:00 to 8:00 and 9:15 to 11:15.
